ABSTRACT

Objective To evaluate the effect of hyperbaric oxygen (HBO) therapy on postoperative cognitive dysfunction (POCD) in elderly patients undergoing general anesthesia.Methods A total of 112 patients,aged 65-75 yr,of American Society of Anesthesiologists physical status Ⅱ or Ⅲ,undergoing elective non-cardiac surgery with general anesthesia,were randomly divided into control group (C group,n =54) and HBO group (n =58).Patients were exposed to hyperbaric oxygen in a hyperbaric oxygen chamber once a day from day 3 to day 12 after surgery in both groups.Pressure was slowly increased to 2 atmosphere absolute within 20 min,pure oxygen was inhaled for 35 min by mask,5 min later pure oxygen was inhaled for another 35 min,oxygen inhalation was then stopped and pressure was slowly increased to 1 atmosphere absolute in HBO group.Patients inhaled air at 1 atmosphere absolute for 70 min in C group.Cognitive function score was assessed using Mini-Mental State Examination,language ability test,visual identification function test,digit span backwards task and Hasegawa's Dementia Scale (HDS) at 2 days before surgery and 7 and 13 days after surgery.The development of POCD was recorded.Results Compared with the baseline at 2 days before surgery,language ability test,digit span backwards task and HDS scores were significantly decreased at 7 and 13 days after surgery in C group,and digit span backwards task scores were significantly decreased at 7 days after surgery in HBO group (P<0.05 or 0.01).The language ability test and HDS scores were significantly higher,and the incidence of POCD was lower at 7 and 13 days after surgery in HBO group than in C group (P<0.05).Conclusion HBO therapy can reduce POCD in elderly patients undergoing general anesthesia.

ABSTRACT

We developed the monoclonal antibodies against nucleoprotein (NP) of Newcastle disease virus (NDV),and established a double antibody sandwich ELISA method for quantitative determination of NP antigen of NDV (NDV NP ELISA).The recombination NP protein derived from strain F48E9 of NDV were prepared and used to immunize BLAB/c mice.The mouse splenic cells from immunized mice were fused with SP2/0 cells to generate monoclonal antibodies (mAb).The NDV NP specific mAbs were paired to establish a double antibody sandwich ELISA method.The performance of the NDV NP ELISA was evaluated,including specificity,sensitivity,precision,accuracy and linearity.The correlation between the ELISA and PFU virus titer was analyzed by regression analysis method.Two monoclonal antibodies 3C10 and 4E7 were selected to establish double antibody sandwich ELISA for NP antigen of NDV.The linearity and performance of the NDV NP ELISA was characterized.The detection linearity fell in the range of 0.015-0.250 μg/mL (R2 =0.997 4).The detection limit of the assay was 0.015 μg/mL.The recovery was between 88.4% and 106.01%;the variation coefficient was below 3.4%.In testing of 50 NDV virus samples,this assay performed well and correlated comparably with PFU virus titer (R2 =0.920 9).The NDV NP ELISA for quantitative detection of NDV is a reliable quantifiable assay for detection of NDV NP protein;it provides a new approach for rapid and quantitative detection of Newcastle disease virus.

ABSTRACT

ObjectiveToinvestigatetheriskfactorsforunfavorableoutcomeinpatientswithminor ischemic stroke. Methods Patients with minor ischemic stroke were enroled prospectively. The modified Rankin Scale ( mRS ) w as used to assess the clinical outcome at day 90 after onset, and mRS 0-2 w as defined as favorable outcome. The demographic data, vascular risk factors, clinical data, imaging data, stroke etiologic subtypes, laboratory test results, and treatment methods in the favorable outcome group and unfavorable outcome group w ere compared. Multivariate logistic regression analysis w as used to identify the independent risk factors for early poor outcome in patients w ith minor ischemic stroke. Results A total of 516 patients with minor ischemic stroke were enroled. At day 90 after onset, 90 patients (17.44%) had unfavorable outcome and 426 (82.56%) had favorable outcome. Multivariate logistic regression analysis showed that age (odds ratio [OR] 1.045, 95% confidence interval [CI] 1.017-1.074; P=0.002), heart diseases (OR 2.021, 95%CI 1.063-3.841; P=0.032), baseline National Institutes of Health Stroke Scale (NIHSS) score (OR 1.662, 95%CI 1.177-2.347; P=0.004), limb movement disorder ( OR 2.430, 95%CI 1.010-5.850; P=0.048), ataxia (OR 2.929, 95%CI 1.188-7.221;P=0.020), early neurological deterioration ( OR 50.994, 95%CI 17.659-147.258; P<0.001), infarct diameter ( OR 1.279, 95%CI 1.075-1.521; P=0.005), non-responsible vascular stenosis ( OR 2.518, 95%CI 1.145-5.536;P=0.022), and large artery atherosclerotic stroke ( OR 2.010, 95%CI 1.009-4.003; P=0.047) w ere the independent risk factors for unfavorable outcome in minor ischemic stroke. Conclusions The early poor outcome of minor ischemic stroke is closely associated w ith age, heart diseases, baseline NIHSS score, limb movement disorder, ataxia, early neurological deterioration, infarct diameter, non-responsible vascular stenosis, and large artery atherosclerotic stroke. The relevant examinations need to be improved early, the etiologic subtype should be identified, and the correct clinical treatment should be guided.

ABSTRACT

After signs of risk have been happened in risk management of medical equipment at present, reports are taken step by step. So there is a report not timely, incomplete information, it is difficult to monitor, and many other problems. With the improvement of risk management requirements; the development of the information technology s apply, and increasing sources of information used for risk early warning analysis. This paper analyzes the requirement of risk management, and proposes a total solution of enterprise risk early warning based on EAI. It will make managers accurately and fully grasp the risks, find risk signs timely, speed up the response to risk.

ABSTRACT

Objective To evaluate the application of interferon-γ release assay T-SPOT.TB in diagnosis and efficacy assessment of pulmonary tuberculosis. Methods T-SPOT.TB assay was used to determine spot-forming cells (SFCs) formed by T-cells when stimulated by Mycobacterium tuberculosisspecific antigens in 55 patients with active tuberculosis,14 patients with non-tuberculosis lung diseases and 12 healthy controls. Meanwhile 20 sputum culture-positive and qualitative assay-positive pulmonary tuberculosis patients were tested with T-SPOT.TB before and at 2-month and 6-month after treatment.Kruskal-Wallis H and Mann-Whitney U test were used in group comparison.Wilcoxon test was used in comparison between pre- and post-treatment.Results The positive rate of T-SPOT.TB was significantly higher in patients with tuberculosis (85.5%,47/55 ) than that in patients with non-tuberculosis lung diseases (2/14) and the healthy controls (1/12) (x2 =40.926,P <0.05).The SFCs of hole A in response to ESAT-6 and hole B in response to CFP-10 in pulmonary tuberculosis group were 70.00 (27.00 -125.00) and 80.00 ( 17.00 - 180.00),respectively,which were all significantly higher than those in nontuberculosis lung diseases group and the healthy controls (x2 =35.376 and 30.485,P < 0.05 ).The sensitivity,specificity,positive predictive value and negative predictive value of T-SPOT.TB in diagnosis of smear-positive tuberculosis were 88.6%,88.5%,91.2% and 85%,while in diagnosis of sputum smearnegative tuberculosis,the sensitivity was 80%,specificity was 88.5%,positive predictive value was 84.2% and negative predictive value was 85.2% ( P > 0.05 ).SFCs of hole A and hole B in 20 patients with sputum culture-positive and qualitative assay-positive pulmonary tuberculosis were 75.50 (41.25 -116.25 ) and 56.25 ( 105.00 -225.00) before the treatment.After 2-month antituberculosis treatment,the SFCsofhole A and hole B were 41.0 (18.0-68.75) and 72.50 (42.25- 158.75),which were significantly lower than those before treatment (Z =- 3.213 and - 3.622,P < 0.05 ).Ater 6-month antituberculosis treatment,the SFCs of hole A and hole B were 25.00 (5.75 - 52.25) and 55.00 (6.25 -122.50),which were significantly lower than those before and 2-month after antituberculosis treatment (vs.before treatment:Z =- 3.921 and - 3.923,P < 0.05 ; vs.2-month antituberculosis treatment:Z =- 3.926 and - 3.884,P < 0.05 ).Conclusions T-SPOT.TB assay possess satisfactory sensitivity and specificity in diagnosis of tuberculosis infection,especially for sputum-negative pulmonary tuberculosis.It is also of value in monitoring antituberculosis treatment.

<p><b>OBJECTIVE</b>This paper adopts advanced information technology to manage medical supplies, in order to improve the medical supplies management level and reduce material cost.</p><p><b>METHODS</b>It develops a Medical Supplies Management System with B/S and C/S mixed structure, optimizing material management process, building large equipment performance evaluation model, providing interface solution with HIS, and realizing real-time information briefing of high value material's consumption.</p><p><b>CONCLUSION</b>The medical materials are managed during its full life-cycle. The material consumption of the clinical departments is monitored real-timely. Through the closed-loop management with pre-event budget, mid-event control and after-event analysis, it realizes the final purpose of management yielding benefit.</p>

ABSTRACT

ObjectiveTo investigate the relationship between life meaning and life satisfaction among university students.MethodsTotally 473 university students were investigated with The Purpose In Life Test and Life Satisfaction Scale.Results ( 1 ) The scores of life satisfaction and positive affection in highcr levels of life meaning were significantly higher than those in the lower levels of life meaning( (4.53 ± 1.04)vs(3.38 ± 0.98 ),(2.89 ±0.88)vs(2.04 ± 1.07) ) among the university students(F=27.806,P<0.01 ; F=13.635,P< 0.01 ),however the higher levels of life meaning were significantly lower than the lower levels of life meaning ( ( 1.09 ± 1.12 ) vs ( 2.45 ± 1.41 ) ) in negative affection (F =21.513,P < 0.01 ).( 2 ) The students' life satisfaction was negatively correlated with escape( r=-0.336,P<0.01 ).The others factors of life meaning were positively correlated with life satisfaction ( r =0.285 ~ 0.380,P < 0.01 ).The negative affection was negatively correlated with life meaning( r =-0.121 ~ -0.335,P < 0.05 ).(3)The escape had significantly negative prediction on life satisfaction and positive affection ( β =-0.133 ~ -0.179,P < 0.05 ).The life of goals and the life of passion had significantly positive prediction on life satisfaction and positive affection ( β =0.170 ~ 0.206,P < 0.05 ).Conclusion Life meaning of the university students is closely related to life satisfaction,the higher levels of life meaning can upgrade individual life satisfaction.

ABSTRACT

OBJECTIVE To investigate the antimicrobial-resistant genes in meticillin-resistant Staphylococcus aureus(MRSA).METHODS Forty strains of MRSA were clinically isolated.Nine kinds of antimicrobial-resistant genes were analyzed by polymerase chain reaction(PCR).RESULTS The positive rates of mecA,aac(6′)/aph(2′)and tetM were all 100.0%.The positive rates of ermA/B/C,aph(3′)-Ⅲ,qacA/B,PVL and ant(4′,4″) were 82.5%,67.5%,45.0%,10.0% and 2.5%,respectively.CONCLUSIONS There are higher positive percentages of relevant resistance genes in MRSA isolates clinically.MRSA can induce clone transmition in hospital which should pay,and attention to the disinfection and isolation.

ABSTRACT

OBJECTIVE To investigate the prevalence of sulfamethoxazole/trimethoprim(SXT)-resistance genes and drug resistance in Escherichia coli isolates from the old patients.METHODS Drug resistance of 20 E.coli strains were analyzed.The SXT-resistance genes(sul1,dfrA1,dfrA12 and dfrA17)were detected by polymerase chain reaction(PCR).The PCR products were purified and sequenced.RESULTS In 20 E.coli strains,95.0% were SXT resistant.The positive rate of genes of sul1,dfrA12 and dfrA17 were 80.0%,20.0% and 65.0%.Gene of dfrA1 was not detected out,17 strains of E.coli were found dfrA gene.The SXT-resistance gene dfrA or sul1 was detected in 18 of the 20 strains.CONCLUSIONS The SXT resistance of E.coli can be caused mainly by gene expression of dfrA and sul1,and the genes of sul1,dfrA12 and dfrA17 are all firstly found and reported in China.

ABSTRACT

OBJECTIVE To investigate the existance of class Ⅰ gene integron and transposon(merA and tnpA) gene marker in Escherichia coli isolated from the elderly patients.METHODS Antibiotic sensitivity was performed by microdilution test.Integron gene IntⅠ1 and transposon marker genes were detected by polymerase chain reaction(PCR).The PCR products were purified and sequenced.RESULTS E.coli resistant to amikacin accounted for 60%,to sulfamethoxazole 95%,and to third-generation cephalosporin 100%(all ESBLs-producing strains).The resistance was related closely to the class Ⅰ integron.The positive rate of integron intⅠ1 was 80%(16/20),whereas the positive rate of gene merA was only 10%(2/20).Gene tnpA was not tested out.CONCLUSIONS The drug resistance of E.coli has close relation with transmission of integron and transposon.The transposons of Tn21/Tn501 are all firstly reported in China.

ABSTRACT

OBJECTIVE To investigate the chloramphenicol-resistant gene in Escherichia coli strains isolated from the old patients.METHODS Twenty strains of E.coli were clinically isolated.The resistance genes were analyzed by polymerase chain reaction(PCR) and verified by DNA sequencing.RESULTS The positive rates of catB and cmlA were 20% and 20%,respectively.Two strains of E.coli were found containing catB gene and cmlA gene.CONCLUSIONS The resistance to chloramphenicol in these 20 strains of E.coli has a close relation to antimicrobial-resistance gene existed,and the genes of catB and cmlA in E.coli are all firstly found and reported in China.

ABSTRACT

Objective To improve our knowledge by analyze the image findings of stromal tumor at small intestine.Methods The image findings in 23 cases with pathological-proved small intestinal stromal tumor were retrospectively analyzed as follow:image examine with GI(11 cases),DSA(2 cases),B-us(13 cases) and CT(all cases).Results The tumors originated from the duodenum(n=5),jejunum(n=11) or ileum(n=8).Found by GI(n=2),DSA(n=2),B-us(n=12) and CT(n=21).The lesions were mainly round or oval in shape.On plain CT scans,the tumors showed low-density centrally with isodensity peripherally(n=17),or homogeneous isodensity(n=4),or cystical low-density(n=2).On enhanced CT scans,moderate or marked enhancement was seen in most cases.Conclusion It has some spacial findings by image examination about small intestinal stromal tumor,include:well-defined margins,markedly enhancement,no metastases to local lymph nodes and the common clinical symptoms.CT and B-us are the mainly examination,therefore,a confirmed diagnosis relies on the pathological examination.

ABSTRACT

OBJECTIVE To investigate the ?-lactamases in Klebsiella pneumoniae strains isolated from the elderly patients.METHODS We had an observation on 20 strains of Klebsiella pneumoniae about 15 kinds of ?-lactamase gene test(blaTEM,blaSHV,blaLEN,laOKP,blaCTX-M-1 group,blaCTX-M-2 group,blaCTX-M-9 group,blaOXA-1 group,blaOXA-2 group,blaOXA-10 group,blaGES,blaPER,blaVEB,blaDHA,and blaACT-1).RESULTS Six kinds of ?-lactamase genes had been found out from these 20 strains of K.pneumoniae,namely,blaTEM,blaSHV,blaCTX-M-1 group,blaCTX-M-9 group,blaOXA-1 group,and blaDHA.Their positives were 95%,30%,50%,5%,5% and 15%.Among these 20 strains of K.pneumoniae,there were 19 ones of K.pneumoniae which at least contained 1 kind of ?-lactamase genes.Fifteen strains of K.pneumoniae were found containing more than 2 kinds of ?-lactamase genes,at the same time.Some were even tested out having 4 kinds of ?-lactamase genes.CONCLUSIONS The resistance of ?-lactamase antibiotics of these 20 strains of K.pneumoniae has a close relative to the number of ?-lactamase.

ABSTRACT

OBJECTIVE To investigate the epidemic situation of nonfermentative bacteria in lower respiratory tract infection and guide rational use of antimicrobials.METHODS API system was applied for bacterial identification.K-B disk diffuse method was used in susceptibility testing.WHONET 5.3 software was used for data analysis.Pseudomonas aeruginosa,Acinetobacter baumannii and Stenotrophomonas maltophilia isolates were included in this study.RESULTS Less than 20% of P.aeruginosa isolates were resistant to amikacin,imipenem or cefoperazone/sulbactam.For A.baumannii,the most active compounds tested were imipenem and cefoperazone/sulbactam with resistance rates of 4.8% and 7.3%,respectively.26.6% And 27.4% of A.baumannii isolates were resistant to piperacillin/tazobactam and ticarcillin/clavulanic acid.The resistance rate of S.maltophilia to cefoperazone/sulbactam,ticarcillin/clavulanic acid and piperacillin/tazobactam was 0,2.7% and 10.8%,respectively.CONCLUSIONS The isolation rate of nonfermenters is increasing in the lower respiratory tract infection,and the ways to improve the cure rate are to avert nosocomial infection and choose antimicrobials properly.

ABSTRACT

<p><b>OBJECTIVE</b>To study the existence of natural foci of Spotted Fever in Ninghua, Fujian province.</p><p><b>METHODS</b>Using DNA polymerase chain reaction and restriction endonuclease fragment length polymorphism analysis (PCR/RFLP) to detect spotted fever group rickettsiae (SFGR) in ticks and rodents.</p><p><b>RESULTS</b>It was found that H. wellingtoni, H. yeni, and Dermacentor auratus were infected with Rickettsia sibirica; the DNA fragments were cloned, the PCR products from isolated strain NH-97 were antigenically and genotypically identical to Rickettsia sibirica. Rattus flavipectus were found infected with R. conorii. One of the sequeuce analysis showed that the DNA sequence was different from other SFGR and close to R. japanic.</p><p><b>CONCLUSION</b>Natural foci of R. sibirica, R. sibrica, R. japanic and R.conorii are found in Ninghua, Fujian province of China.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To retrospectively compare combined endovascular intervension with traditional bypass operation for the treatment of Budd-Chiari syndrome (BCS).</p><p><b>METHODS</b>From July 1989 to June 2001, 49 patients undergoing surgery for BCS were studied. 32 operations were performed by traditional bypass (from superior mesenteric vein or inferior vena cava to right auricle), and 17 by combined endovascular operation.</p><p><b>RESULTS</b>The data demonstrated a high incidence of perioperative complications, longer hospital stay, and expensive cost in the former group than in the latter group (P < 0.01). The mid-term effects were significantly better in the latter than in the former (P < 0.05). Severe complications occurred in the bypass group included hepatoencephalopathy, obtinacy ascites, cardiac dysfunction, and embolization of vascular grafts.</p><p><b>CONCLUSIONS</b>Combined endovascular intervention and shunting are the treatment of choice for BCS, with different combination according to its clinical type. This approach is simple, safe, effective, and economic.</p>

ABSTRACT

Objective To study the relative factors of concomitant depression after stroke.Methods Using Geriatric depression Scale(GDS) and SSS,we scored 121 storke cases who were treated from January 2001 to December 2001, in which 54 cases were concomitant with depression, assessed defect and promotion degree of nervous function, and observed relationship between number of lesion and onset of depression.Result Incidence of depression of cerebral hemarrhage patients was apparently higher than that of cerebral infarction patients (P< 0.05), there was no apparent difference in lesion location and concomitant depression (P >0.05), incidence of concomitant depression of stroke patients with multiple lesions was apparently higher than that of stroke patients with single lesion.Conclusion Concomitant depression of stroke patients is related to location, number of lesions and severity of stroke.
